What to Expect On Your First Visit
Prior to your first visit, we will mail or email a patient form created by Dr. Fishkin to gather information about your medical history and the factors affecting your health.
We will also collect information about your insurance so that you’re informed of your coverage.
Depending on your condition, we may ask you to complete an additional form/s prior to your first visit that you can submit online, by fax, or in person.
On your first visit, Dr. Fishkin will consult with you personally to identify and understand your symptoms and discuss treatment options. Our strategy at the Fishkin Center is to explore alternatives to surgery and pain medication.
Insurance Carriers Accepted
We will also be happy to submit bills to other carriers.
We accept most insurance plans in our office. In addition, we will work with auto accidents, workers’ compensation, and personal injury insurers.
Since there are many plans and coverage varies widely, please call us, and we will check your plan coverage details.
The insurance information we provide the patient will only be as accurate as the information provided to our staff at the time of inquiry.
