Exercise and rehabilitation are important components in healing and staying healthy. Dr. Fishkin may supervise you on exercise equipment in the office, teach you how to navigate equipment in your gym, or meet with your trainer.
Dr. Fishkin is a big proponent of stretching to help return flexibility and normal range of motion to injured or painful areas. He routinely stretches the patient during visits and teaches techniques you can do yourself or with the aid of a partner. (edit)
Myofascial trigger point dry needling (MFTrPDN) is a treatment to restore proper function to abnormally behaving muscles that become “locked”, preventing normal muscle function, which causes restriction of movement and chronic pain.
Many methods of treatment are applied to painful muscles but none are quite as effective, fast acting or long lasting as dry needling.
Muscles can become painful with nodules, also called knots, and tight bands of muscle; the pain can be local or send pain signals to other locations in the body.
These knots develop from overuse, trauma, poor posture, lack of exercise, sleep disturbances, vitamin deficiencies, joint arthritis, or nerves being compressed at the spine and other locations. These painful areas contain biochemicals that cause pain to the local nerves, and decreases oxygen flow (called ischemia), causing further pain resulting in muscle fibers to “lock”.
Dry needling is so effective because it short-circuits the vicious cycle in a direct way that other methods cannot accomplish. This allows the muscle to release to its normal length, improves circulation, reduces swelling, and disperses the pain-causing chemicals, thereby removing their noxious effect on local nerves.
The treatment utilizes very thin sterile needles to achieve its goal. The relief is often rapid and the patient becomes relaxed and tired.

Flexion/distraction manipulation is a treatment developed by James M. Cox. It is often used for lumbar disc injuries (herniation, bulges, etc.), and for other low back and lower extremity conditions.
Flexion-distraction involves the use of a specialized table that gently “distracts” or stretches the spine as it is gently rotated. This allows the chiropractor to isolate the affected area while slightly “flexing” the spine using a pumping rhythm. There is no pain associated with this treatment. Instead, the flexion-distraction technique’s gentle pumping to the painful area allows the center of the intervertebral disc (called the nucleus pulposus) to assume its central position in the disc. Flexion-distraction may also improve disc height.
These different planes of motion, along with the use of appropriate adjunctive therapy and exercises, allow for reduction of symptoms attributable to lumbar disc syndromes.
