What to Expect On Your First Visit

Prior to your first visit we will mail or email a patient form created by Dr. Fishkin to gather information about your medical history and factors affecting your health.

We will also collect information about your insurance so that you’re informed of your coverage.

Depending on your condition, we may ask you to complete an additional form/s prior to your first visit that you can submit online, by fax, or in person.

On your first visit, Dr. Fishkin will consult with you personally to identify and understand your symptoms, and to discuss treatment options. It’s our practice at the Fishkin Center to explore alternatives to surgery and pain medication.
